The documentation only mentioned configuring server.cfg. By upstream what
are you referring to? The main proxy?

Did you set up apserver.conf correctly? You need to set the upstream proxy,
and I set username (but not password).
You could use wget to test it, by setting the proxy env var and trying to
use wget to get a known accessible page.
